Waiver of Liability for Gym use, Media, and Services provided.
This release form is for any and all liability for personal injuries (including death), property loss or damage occasioned by, or in connection with the person or entity being released.
I/We hereby understand and acknowledge that the training, programs, events, and services held by Leader Fitness LLC may expose me to many inherent risks, including accidents, injury, illness, or even death.
I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.
I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity.
I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in.
After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and Leader Fitness LLC furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE Leader Fitness LLC, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in any Leader Fitness LLC training, programs, events, or services.
The person or entity consents to Leader Fitness LLC taking photographs or video images of the Member carrying on the Activities or otherwise in the public premises of Leader Fitness LLC (the "Footage") and hereby irrevocably grants Leader Fitness LLC the right to use the person or entity's image or likeness in such images or photographs in any advertising, promotion, marketing materials including any and all social media, including but not limited to YouTube, Facebook, Instagram, print media, or on the Leader Fitness LLC website. The person or entity hereby assigns all rights of every kind and character whatsoever, including without limitation copyright, in and to the results and proceeds of the person or entity's appearance in the Footage. The person or entity hereby waives their moral rights in the Footage in favour of Leader Fitness, its successors, assigns, assignees and licensees
The person or entity further agrees to abide by all the rules and regulations promulgated by the person or entity being released and/or its affiliate groups and vendors by my signature or initials.
I/We indicate that I/We have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.
